Expectations:
Responsible for the timely flow of patients through the department, coordinate patient care plans, perform patient assessments, and provide patient education and follow-up on clinical problems. Maintains department schedule efficiencies for patient and physician. Nursing assessments to include documentation of vital signs and complete review of systems. Ensures that necessary records are present on the chart prior to patient appointment. Provides comprehensive patient care utilizing the nursing process, and medical plan of care within HMH nursing clinical policies and procedures. This position may require flexibility of hours.
EDUCATION:
Bachelor's degree (BSN) required. Associate Degree in Nursing (ADN) will be considered if enrolled in a BSN program with degree completion within 2 years of date of hire or date of transfer to an RN position. Diploma or transcripts on highest degree earned required.
EXPERIENCE/TRAINING:
Minimum 1 year of current experience in acute care area of expertise/equivalent service area, 2 years preferred.
Must challenge and successfully pass the Basic Dysrhythmia course curriculum within 21 days of hire. Basic computer skills/experience with computerized documentation systems required.

Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
Preferred:
Current Certified Radiology Nurse (CRN) or related certification.
SKILLS:
Unit Specific Essential Functions:
1. Contacts new patients prior to consult visit to ensure that all necessary documents are brought to the appointment by the patient or arrangements made for forwarding documents to Radiation Oncology from referring physician.
2. Ensures all medical records, lab reports, radiology reports and films or slides are present on the chart for physician review at the time of consult.
3. Performs initial patient assessment using clinical assessment tool; obtains vital signs and weight.
4. Explains the consult appointment process to the patient.
5. Provides patient education; distributes patient education packet describing patient expectations during treatment and answers questions.
6. Coordinates care with Clinical Research regarding any appropriate protocols.
7. Schedules all head and neck cases for dental prophylaxis for the making of fluoride stents that needs to be brought in at the time of simulation.
8. Directs patient for scheduling of next appointment.
9. Assists physicians to ensure consent form, CT Sim Instructions/worksheet, and H&P dictation are completed on day of consult.
10. Prepares follow-up patient chart; reads previous visit physician notes and obtains test results from recent labs or x-rays.
11. Performs vital signs, obtains patient weight and pain assessment on all follow-up patients.
12. Provides site-specific care and assessment of patient compliance with self-care, medication schedule, and need for new instructions.
13. Instructs patient on dose if new medications are ordered with possible side effects.
14. Provides skin care instructions, refers patients to schedule physician ordered tests, and refers patients to specialists such as care coordinator, social worker, or nutritionist as clinically needed.
15. Ensures accurate and complete chart documentation.
16. Performs weekly patient assessment for patients under treatment. Obtains weight, vital signs, and blood draw as needed. Informs physician of findings prior to physician weekly management visit.

Pay Range: The estimated base rate for this position is $63.11 - $87.57.
Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
