Anticipated compensation range of $105,155.00 – $131,309.00 / Year depending on qualifications and experience.

Expectations:
Under the supervision of the Community Nursing Supervisor/SCN and with direction from the program's Clinical Supervisor, provides case finding, consultation, health-focused assessment/care coordination services and care planning services to adults with disabilities, older adults and their families in various programs. Provides services in home and community-based settings in order to preserve health, maintenance of function and continued independence through assessing needs, monitoring quality of care, and providing education, advocacy, and support. Provides nursing consultation and guidance to departmental staff. This position requires flexibility of hours.
EDUCATION:
Effective for hires after 9/13/2015: Bachelor's degree in nursing (BSN) required. Associate degree in nursing (ADN) will be considered if enrolled in a BSN program with degree completion within 2 years of date of hire or date of transfer to an RN position. Diploma or transcripts on highest degree earned required.
EXPERIENCE/TRAINING:
One year of clinical nursing experience required (preferably with geriatric populations) with at least 1,000 hours in an acute care hospital or 2,000 hours in a home health setting as required by the California Department of Health Care Services.
LICENSES/CERTIFICATIONS:
Required:
Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
Current Basic Life Support Provider (BLS) issued by American Heart Association
SKILLS:
Clinical assessment skills, knowledge of community resources and public benefits, ability to work with a multidisciplinary team and with clients/families in the home setting. Knowledge of aging programs for seniors and/or adults with disabilities. Computer skills including Microsoft Office and ability to learn and utilize specialized software with training and support. Ability to provide nursing consultation and guidance to departmental staff.
